If this is your first visit to our Plan website, you will need to establish your User ID and Password. To do this, enter your e-mail address (the one that was pre-authorized by the Plan Administrator) and the last 4 digits of your Social Security Number (SSN). We will e-mail you a temporary password from [email protected] to get you started.
